This policy covers information collected online. Health information we receive while providing services to clients is protected under HIPAA and governed by our separate Notice of Privacy Practices, available on request.

• Inquiry details: the service you are interested in (such as Attendant Care, Home and Community Assistance, or Structured Family Caregiving), Medicaid waiver type, preferred contact method, and any message you send us.
• Employment applications: work history, certifications, availability, and references submitted through our careers forms.
• Text messaging consent: your opt-in choice, the date and time you consented, and the phone number you enrolled.
• Technical data: IP address, browser type, device type, and pages visited, collected automatically through standard website analytics and cookies.

How We Use Your Information

We use your information only to operate our agency and respond to you. Specifically, we use it to:

• Respond to inquiries and schedule assessments or consultations.
• Coordinate services, including appointment reminders, caregiver scheduling updates, and service notifications.

• Review employment applications and communicate with applicants.
• Meet legal, licensing, and Indiana Medicaid compliance obligations.

Text Messaging (SMS) Program

• How you opt in: by checking the text-messaging consent box on our website forms, or by giving verbal or written consent to our staff.
• What we send: appointment and visit reminders, scheduling updates, service notifications, and responses to your inquiries. For employees and applicants: shift and onboarding updates.
• Frequency: message frequency varies.
• Cost: message and data rates may apply.
• Opt out: reply STOP to any message to unsubscribe. You will receive one confirmation and no further texts.
• Help: reply HELP for assistance, or call us at 317-737-1104.
• Not a condition of service: consent to receive text messages is not required to receive services or to apply for employment.

Carriers are not liable for delayed or undelivered messages.

How We Share Your Information

We share personal information only in these limited cases:

• Service providers: vendors that help us run our website, forms, and messaging platform. They may use your information only to perform services for us and must keep it confidential. This does not include sharing SMS opt-in data or consent for their own marketing purposes.
• Care coordination: with your case manager, Medicaid waiver program, or care team when needed to arrange services you requested, and as permitted under HIPAA.
• Legal requirements: when required by law, subpoena, licensing authority, or to protect the safety of a client or others.
